Background

The Food Allergy Quality of Life Questionnaires (FAQLQ) are disease-specific health-related quality of life (HRQL) questionnaires for patients with food allergy. The FAQLQs are reliable and valid instruments to measure the impact of food allergy on HRQL. There are different forms for specific groups.

Forms that are completed by the patients themselves:

• FAQLQ-CF, Child Form (8-12 years)
• FAQLQ-TF, Teenager Form (13-17 years)
• FAQLQ-AF, Adult Form (18 years and older)

Forms that are completed by parents of food allergic children and teenagers:

• FAQLQ-PF, Parent Form  (0-12 years)
• FAQLQ-PFT, Parent Form Teenager  (13-17 years)
